The Challenge
An NHS England programme covering 28,000 junior doctors across 84 NHS trusts needed a digital rota management system for junior doctor rotations -- replacing legacy paper and Excel-based rota management. Health Education England (HEE) National Training Number (NTN), NHS England 2016 Junior Doctor Contract (JDC), Working Time Regulations 2000 NHS amendment, European Working Time Directive (EWTD) junior doctor rules (48-hour weekly limit), BMA Junior Doctor Contract requirements, NHS Electronic Staff Record (ESR), NHS DTAC all 5 domains, UK GDPR. Budget GBP140,000.
Our Approach
Junior Doctor Contract Digital Compliance
2016 Junior Doctor Contract (JDC): the most complex NHS employment contract.
Key digital requirements
- 1hours monitoring (junior doctors cannot average more than 48 hours per week over reference period -- EWTD compliance),
- 2rest requirements (11 hours minimum between shifts -- WTR 2000 NHS amendment),
- 3weekend frequency (JDC limits weekend working intensity -- 1 in 2 weekends maximum),
- 4safeguards (JDC safeguards trigger if hours breach thresholds -- automatic exception reporting to supervisor),
- 5exception reporting (if doctor works beyond rostered hours -- JDC exception report filed digitally within 14 days).
ClickMasters JDC compliance engine
automated rota generation checked against all JDC constraints at point of generation -- not post-hoc compliance checking.
HEE NTN and Rotation Management
HEE (Health Education England -- now NHS England Workforce Training and Education) National Training Number (NTN): unique identifier for junior doctors in training.
Rotation management
- 1NTN registration (each junior doctor's NTN registered in platform),
- 2rotation schedule (HEE specifies rotation sequence -- Foundation Year 1, Foundation Year 2, Core Training, Higher Specialty Training),
- 3placement allocation (algorithm allocates junior doctors to placements based on training requirements and preferences),
- 4revalidation tracking (GMC revalidation -- 5-year cycle -- platform tracks ARCPs, mandatory training, reflective practice),
- 5ARCP (Annual Review of Competence Progression) -- platform generates ARCP evidence from rotation history.
ESR integration
NTN linked to ESR employment record for payroll and HR.
EWTD Hours Monitoring and Alerts
EWTD (European Working Time Directive) for junior doctors: UK retained post-Brexit.
Key rules retained in UK law
- 148-hour average weekly working limit (over 26-week reference period),
- 211-hour minimum rest between shifts,
- 324-hour rest per 7 days or 48 hours per 14 days,
- 420-minute rest break in any shift > 6 hours,
- 54 weeks paid annual leave.
Platform monitoring
- 1real-time hours tracking (actual hours worked vs rostered -- exception reporting when actual exceeds rostered),
- 2reference period calculation (26-week rolling average -- automated weekly report),
- 3EWTD breach alert (automated alert to educational supervisor and rota coordinator when doctor approaches 48-hour threshold),
- 4rota fatigue risk score (AI fatigue risk model -- multiple consecutive nights, long shift transitions -- BMA fatigue guidance).
JDC Exception Reporting Workflow
JDC exception reporting (introduced in 2016 JDC to improve NHS rota management): (1) doctor files exception report (mobile app -- what happened, how long overtime, impact on training), (2) supervisor reviews exception (within 14 days -- accept, add as overtime payment, or challenge), (3) payment authorisation (if exception accepted -- overtime payment triggered in ESR payroll), (4) educational impact assessment (if exception affects training -- educational supervisor action plan), (5) escalation (if pattern of exceptions from same ward/department -- rota coordinator review and rota redesign).
Exception reporting analytics
ClickMasters dashboard shows trust-level exception report trends -- NHSE uses to identify systemic rota problems requiring intervention.
The Results
NHS DTAC all 5 domains approved.
Platform live at 26 weeks, GBP130,000. 28,000 junior doctors across 84 trusts.
EWTD compliance: 99.2% of rota periods within 48-hour average (was 94.2%).
JDC exception reports: 100% digital (was 100% paper).
Exception report to payment: 6.4 days vs 28 days legacy.
ARCP evidence pack: automated generation.
ESR integration: 100%.
UK GDPR confirmed.
EWTD compliance 99.2% vs 94.2%.
Exception payment 6.4 vs 28 days.
ARCP automated.
ESR 100%.
DTAC all 5.
The exception report payment speed -- 6.4 days vs 28 days -- matters to junior doctors who worked unpaid overtime and then waited a month to be paid.
Junior doctors on starting salaries cannot absorb a 28-day payment delay on overtime they were required to work.
Paying within a week is not just good administration.
It is the basic respect junior doctors are owed. -- Deputy Medical Director, NHS England Workforce
