The Challenge
A UK digital mental health company wanted to build a self-guided CBT (Cognitive Behavioural Therapy) app for mild-to-moderate anxiety and depression, integrated with NHS Talking Therapies (formerly IAPT — Improving Access to Psychological Therapies). NICE guidelines for digital CBT (DClinPsy-supervised), DTAC all 5 domains, NHS Talking Therapies outcome measures (PHQ-9 and GAD-7 questionnaires), MHRA Class I medical device registration, and UK GDPR Article 9 for mental health data were mandatory. Budget: £80,000.
Our Approach
Compliant Digital CBT Content
- NICE guideline CG91 (depression) and CG113 (generalised anxiety): digital CBT must be based on established CBT protocols.
- Content developed under supervision of DClinPsy (Doctoral Clinical Psychologist) — evidenced in DTAC Domain 1.
Content modules
psychoeducation, cognitive restructuring, behavioural activation, relaxation techniques, relapse prevention.
Safety planning
crisis resources prominently available at all times — not buried in settings.

- self-referral submission to local IAPT service (the app supplements, not replaces, NHS therapy).
- PHQ-9 (depression) and GAD-7 (anxiety) outcome measures: administered in-app weekly, data shared with NHS Talking Therapies clinical record via FHIR R4 QuestionnaireResponse.

- patient identity and consent for data sharing with NHS Talking Therapies service.

Lawful basis
explicit consent (not public task — this is a commercial app with NHS integration).
Granular consent
separate consent for each data category (CBT exercise data, outcome measure scores, sharing with NHS Talking Therapies).
Right to erasure
- immediate deletion of all mental health data on request — with clear explanation that NHS Talking Therapies data may be retained separately by NHS.
- AES-256 field-level encryption for all Article 9 data.

NHS Talking Therapies integration: live in 12 CCG/ICB areas at launch. 4,200 app users in 6 months.
PHQ-9 improvement (reduction ≥5 points): 64% of users completing 8-week programme.
Safety monitoring: 34 safety alerts generated — all actioned with crisis resource delivery.
NICE evidence base review: commended by NHS England digital mental health team.
