The Challenge
A UK community healthcare provider delivering care to 4,800 housebound patients needed a remote patient monitoring (RPM) platform — enabling community nurses to monitor vitals (blood pressure, heart rate, oxygen saturation, weight) between home visits, and detect deterioration early to prevent hospital admission. NHS DSP Toolkit, DTAC all 5 domains, DCB0129 clinical safety, UK GDPR Article 9, MHRA Class IIa device assessment, and NHS Spine integration (PDS for patient demographics) were mandatory. Budget: £90,000.
Our Approach
Bluetooth medical devices
Omron blood pressure monitors (BLE GATT profile), Nonin oximeters (Nonin SDK), Withings scales (Withings API), and Garmin wearables (Garmin Connect API) as supplementary activity data.
React Native patient app
device pairing wizard (Bluetooth permissions, GDPR consent), reading submission, and symptom questionnaire.
Background Bluetooth
readings auto-submitted when patient is at home.
Clinical Deterioration Algorithm
NEWS2 (National Early Warning Score 2): NHS England's standard deterioration scoring system.
Aggregate score from
- respiratory rate, oxygen saturation, supplemental oxygen, systolic blood pressure, pulse rate, consciousness, temperature.
- NEWS2 score → alert tier: 0–4 (routine), 5–6 (nurse review within 1 hour), 7+ (emergency escalation within 30 minutes).
- Algorithm validated against NHS England's published NEWS2 scoring tables.

Community Nursing Senior Sister (registered nurse with community care experience).
Highest hazard
- false negative deterioration detection (algorithm misses deterioration) — mitigated by: NEWS2 algorithm validated against published evidence base, clinical override (nurse can flag concern independent of NEWS2 score), and daily review dashboard (nurse checks all patients regardless of score).
- False positive (unnecessary alarm) mitigated by: 2-reading confirmation before escalation alert.

- NHS Personal Demographics Service (PDS FHIR R4): patient demographics retrieved by NHS Number — name, address, GP.
