The Challenge
An NHS Integrated Care System managing pathology results for 14 GP practices and 2 community hospitals needed to replace the legacy CITO (legacy pathology results viewer, 2008) with a modern pathology results platform — providing clinicians with real-time results, critical value alerting, and trend analysis. HL7 v2 to FHIR R4 migration, NHS Spine integration (SDS for authentication), DTAC all 5 domains, DCB0129 (critical value alerting is a patient safety priority), and UK GDPR Article 9 were the requirements. Budget: £75,000.
Our Approach
NHS Pathology FHIR R4 Implementation Guide
- DiagnosticReport resource (report header — requesting clinician, specimen, report status), Observation resources (individual test results — LOINC codes, numeric values, reference ranges, abnormal flags).
- HL7 v2 ORU R01 →
FHIR R4 transformation
HAPI FHIR message parser with UK Core profile validation.
Bidirectional
receive results (HL7 v2 in, FHIR R4 out for viewer) and send orders (FHIR R4 ServiceRequest out).
Critical Value Alerting
- Critical values (results outside limits requiring urgent clinical action — e.g., potassium >
- 6.0 mmol/L, haemoglobin <
- 70 g/L, INR >
- 10): automatic alert to requesting clinician within 5 minutes of result receipt.
Alert channel
GOV.UK Notify SMS to clinician mobile + in-app notification.
Acknowledgement required
clinician must acknowledge critical alert within 30 minutes — escalation to duty clinician if no acknowledgement.
DCB0129 hazard
alert not delivered or acknowledged — mitigated by dual-channel plus escalation.
NHS SDS Authentication
NHS Spine Directory Service (SDS): GP clinical system authentication for NHS Spine access.
Smartcard integration
NHS Spine requires Smartcard (RA01/02) authentication for clinical data access.
RBAC
NHS Spine Role Based Access Control — GP sees only results for their registered patients (ODS code matching).
Audit
every result access logged with clinician NHS number and ODS code — NHS DSP Toolkit audit trail requirement.
Pathology results
health data, Article 9 special category.
Basis
Article 9(2)(h) — provision of health care.
Retention
pathology results retained for 8 years (NHS Records Management Code — adults).
Minimum necessary access
clinicians see only results for their registered patients.
Results search
clinician cannot search by diagnosis or result value across population — only patient-level access.
The Results
DTAC approved all 5 domains.
Platform live at 14 weeks, £68,000 — under budget.
Critical value alert delivery: 100% within 5 minutes.
Acknowledgement compliance: 94% within 30 minutes (target: 90%).
Clinical result retrieval time: CITO average 4.1 minutes → new platform 18 seconds.
Abnormal result trend analysis: 340 cases where trend identified before individual result crossed critical threshold.
WCAG 2.1 AA: zero non-compliances.
“100% critical value alert delivery within 5 minutes. On the old system, critical results were phoned through — and phone calls were sometimes missed. 340 trend-detected cases where the alert fired before any individual result crossed the critical threshold. DTAC first submission. Result retrieval 4 minutes to 18 seconds — clinicians spend the time saved on patients, not the system." — Chief Medical Information Officer, NHS ICS (name withheld)”
