The Challenge
NHS England's Community Pharmacy Contractual Framework (CPCF) requires community pharmacies to deliver clinical services — Hypertension Case Finding, Contraception Service, Blood Pressure Check, NHS Flu Vaccination, and Pharmacy First (minor ailments scheme from February 2024). A national pharmacy chain with 640 branches needed a platform to manage clinical service delivery, NHS claim submission, and clinical outcome recording. NHS EPS (Electronic Prescription Service), NHS Spine PDS, CPCF claim API, DTAC, UK GDPR Article 9, and WCAG 2.1 AA were mandatory. Budget: £100,000.
Our Approach
CPCF Clinical Service Workflow: Pharmacy First (February 2024): seven clinical pathways (UTI, shingles, sore throat, impetigo, infected insect bite, sinusitis, earache). Consultation workflow: patient presents → pharmacist selects pathway → structured consultation (SOAP format — Subjective, Objective, Assessment, Plan) → treatment recommendation (supply OTC, supply POM under Patient Group Direction, or refer to GP). NHS claim: successful consultation generates Pharmacy First claim submitted to NHS BSA (Business Services Authority) via CPCF claim API. NHS Spine Integration for Pharmacy: NHS EPS (Electronic Prescription Service): prescribed items dispensed against EPS prescription → claim submitted via EPS Release 2. PDS (Patient Demographic Service): patient NHS number lookup from name/DOB for clinical record. SCR (Summary Care Record): pharmacist views patient allergies and current medications from SCR before supplying. NHS ODS: pharmacy ODS code used for all NHS claims. NHS Spine integration: all via NHS Spine IM1 interface (SOAP XML — legacy NHS messaging standard) for EPS/SCR. NHS BSA CPCF Claims Automation: NHS Business Services Authority (BSA): all NHS pharmacy remuneration claims submitted to BSA monthly. CPCF claim types: Pharmacy First consultation claim (per consultation), Hypertension Case Finding claim (per blood pressure check with referral outcome), Flu Vaccination claim (per vaccination administered). Claim submission: automated XML claim submission to NHS BSA Electronic Submission system. Claim reconciliation: BSA payment confirmation matched against submitted claims — discrepancies flagged for manual review. Pharmacist Clinical Documentation: Consultation record: structured SOAP note, clinical findings (blood pressure reading, symptoms checklist), treatment supplied (dm+d medicinal product code), outcome (treated/referred/signposted). SNOMED CT: clinical findings coded to SNOMED CT UK drug extension (pharmacist consultation codes). GPhC (General Pharmaceutical Council) CPD: consultation records exportable for GPhC CPD portfolio. UK GDPR Article 9: health data — pharmacy as controller, NHS BSA as processor for claims. Retention: 8 years (NHS records management code).
The Results
DTAC approved all 5 domains. Platform live at 16 weeks, £92,000 — under budget. 640 pharmacies onboarded in 8 weeks. Pharmacy First consultations processed in first 3 months: 42,000. NHS BSA claims submitted: 100% automated (zero manual claim submissions). Claim rejection rate: 0.8% (industry benchmark: 4.2% — NHS BSA cited platform data quality as exemplary). CPCF contract compliance audit: zero findings. WCAG 2.1 AA: zero non-compliances.
