The Challenge
NHS IAPT service (28,000 referrals/year, 840 therapists, 12 services across 3 trusts) needed to modernise patient pathway management -- replacing paper waiting lists, manual outcome measures, and Excel NHSE reporting. NHS IAPT DDS, PHQ-9/GAD-7/WEMWBS outcome measures, NHSE MHSDS v6.0 monthly submission, NHS DTAC all 5 domains, DCB0129, UK GDPR Article 9 (mental health special category). Budget GBP130,000.
Our Approach
IAPT Digital Patient Pathway
- 1Referral (NHS eRS or self-referral),
- 2assessment (PHQ-9, GAD-7 at intake, problem descriptor, step allocation),
- 3waiting list management (clinical urgency, 18-week tracking),
- 4treatment (Step 2 low intensity vs Step 3 high intensity),
- 5outcome collection (PHQ-9 and GAD-7 every session via GOV.UK Notify). NHS Login P5 for patient self-service. Mental health data in separate encrypted schema -- PostgreSQL RLS per therapist.

patient moves from above PHQ-9/GAD-7 clinical threshold to below, or shows 50% improvement.

- automated MHSDS XML monthly, validated against NHS Digital schema, submitted via NHS MESH.
- Every submission validated before sending -- errors returned to data quality manager.
