The Challenge
A UK private GP network with 12 practices, 84 GPs, and 180,000 registered patients needed a unified digital platform — replacing phone-based appointment booking, paper prescription requests, and fragmented clinical notes. GMC (General Medical Council) clinical record standards, CQC registration (independent healthcare provider), UK GDPR Article 9 (health data), ICO guidance on health data, private prescription regulations (HM Government Human Medicines Regulations 2012), and WCAG 2.1 AA were mandatory. Budget: £110,000.

GOV.UK Notify SMS 24 hours before appointment.
DNA management
no-show tracking, automated rebooking offer, DNA policy (after 3 DNA without contact — patient warned).
HM Government Human Medicines Regulations 2012
private prescriptions require: (1) prescriber name, address, GMC number, (2) patient name, address, date of birth, (3) drug name (generic or brand), dose, quantity, (4) prescriber signature.
Digital private prescription
GP completes structured prescription form → PDF generated (prescription format compliant with HMR 2012) → signed digitally (DocuSign — qualified electronic signature for GMC requirements) → patient receives prescription PDF → patient takes to any pharmacy.
Controlled drugs
CD prescriptions require additional fields (maximum 28-day quantity, no electronic signature — paper required).
GMC Good Medical Practice
clinical records must be clear, accurate, legible, and contemporaneous.
Digital clinical record
SOAP notes (Subjective, Objective, Assessment, Plan) structured entry, read codes (Clinical Terms Version 3 — UK primary care standard), allergy and adverse reaction recording (mandatory), and medication history (BNF drug lookup — NHS Business Services Authority BNF API).
Clinical audit
random 5% clinical record audit (GP Partners review notes for GMC standards compliance).
Records retention
private GP records retained minimum 10 years post-last-consultation (BMA guidance).
CQC Registration and Quality Evidence
- CQC (Care Quality Commission): independent GP practices require CQC registration (regulated activity: diagnostic and screening procedures, treatment of disease).
- CQC Key Lines of Enquiry (KLOEs): Safe, Effective, Caring, Responsive, Well-led.
